We have partnered with Greenspace Health and Videra Health to use their platform to help providers and care teams to track and monitor a patient’s mental health status outside of in-person or live telehealth provider visits. Since almost 40% of patients who suffer from a chronic physical condition also suffer from an underlying mental or behavioral health disorder, our approach to “whole health” monitoring, both physical and behavioral, results in fewer ED visits, more proactive care, better patient outcomes, and higher patient recapture. 

 Without additional staff or clinical resources, these tools provide standard and customized, evidence based assessments for anxiety, depression, mood disorders, substance abuse, and many other behavioral health conditions. This data analyzes these patient scores to identify who is struggling and needs additional attention. Currently, the platform is used to improve patient intake, justify additional treatment during utilization review, track patient progress between levels of care, and improve outcomes through sophisticated post-discharge follow-up. 

Our platform delivers brief, automated check-ins that assess changes in a patient’s health status, even after they leave a facility. This is especially important during the first 30 days of discharge when the likelihood of relapse is highest. Through advanced analytics, the platform delivers actionable alerts to providers, helping triage the patients who are struggling the most and allowing providers to intervene proactively. This powerful shift away from reactive healthcare improves patient satisfaction, reduces readmission rates, helps to meet HEDIS and Star measures, and reduces the total cost of care. 

This platform also includes automated medication adherence programs that continuously monitor your patients, helping ensure they are staying on track with their treatment plan and receiving the full therapeutic benefits they need.
